Using the Informed Mindset Medical™ app, you report, store, update and maintain your personal health information including pain symptoms. You can share your health information digitally with any participating healthcare provider at any time. No more forms to fill out, no more gaps in your health history, no more questions about your current health condition. Enter your health history and contact information once and update chronic conditions regularly. You inform the doctor about your health so the Doctor can efficiently begin diagnosis and treatment.
Entering patient demographic, insurance, medication, and medical history information is easy and intuitive. Your current condition and pain levels can be updated as needed or as requested by your healthcare provider. You can electronically share or update your health information at any time allowing your provider to monitor your condition and to prepare in advance for your appointment. When you arrive for your appointment, you present your smart phone with your secure, unique Quick Response (QR) Code to check-in, eliminating most of the intake paperwork typically requested..
Mindset Medical, LLC contracts with healthcare providers to accept the Informed Mindset Medical™ patient provided personal healthcare information through a proprietary provider application. The Informed provider application allows your healthcare provider to communicate privately and directly to you, to collect additional information and to monitor outcomes.
